3-1: BELLY OF THE BEAST is the first level in Gluttony and the penultimate level of Act I. It is a gauntlet designed to test the player's skills, containing every common enemy the player has encountered so far. 3-1 also holds the entrance to Act I's Prime Sanctum: P-1: SOUL SURVIVOR.
The songs used within the level are Guts and Glory.
Level
Start
Starting off the level, the first room is a small room with various piles of flesh, pillars of tendrils, and decomposing bodies. At the far end lies a door made out of teeth and jaw, leading to a narrow corridor where a single Malicious Face will spawn in front of the player
The door leading afterwards leads into a large room comprised of body parts scattered throughout, and containing a lake of acid, that will constantly deal damage to the player and enemies that come into contact. This room's checkpoint is supported by fleshy growths protruding from the floor that explode in blood when the checkpoint is collected, recovering 60 health to the player.
Walking forward will spawn in a wave consisting of a Mindflayer and four Strays. Killing the Mindflayer will spawn in 2 Streetcleaners, and killing the Strays will awake a dormant Cerberus in the corner of the room.
Once all enemies are dead, the doors will open and the player can proceed, passing through a tooth-lined shaft to the next arena.
Triple Cerberus Room
Dropping down into the room lies a platform of flesh, held up by a set of stone hands over a lake of stale blood, which the player cannot heal from. A small bridge of bone and vertebrae leads to the room's exit.
At the middle of the platform waits three dormant Cerberi. Approaching the exit will wake the two nearest Cerberi, and killing at least one Cerberus will awaken the third.
Bone Walkways
Leading after the room is a large, winding bone staircase, that will spawn various waves of enemies as the player moves forward.
First starting off with 6 Filth, taking the right turn will then spawn 4 Strays. Walking forward, and descending the staircase to the right will spawn 1 Soldier and 4 Strays on the platform where the player originally entered from and 4 Filth on the walkway forward. Continuing on will spawn 3 Drones, which will then turn to an encounter with 4 Streetcleaners. At the end of the stairway, 6 Soldiers will then be summoned by the exit.
Uphill Battle
After the stairway lies a large arena, with a central bridge surrounded by platforms and ramps of flesh. Entering it will summon a wave of 5 Soldiers, 3 Drones and 3 Streetcleaners. Killing them afterwards will spawn 3 Malicious Faces, which once defeated will unlock the doors afterward. After the bridge lies a steep upwards slope, covered with acid. Climbing upwards will spawn a group of 3 Soldiers on a platform to the right, and making it to the exit gate will summon 3 Strays by the door. The checkpoint at the end of this room is supported by flesh that explodes and heals the player for 60 health when the checkpoint is collected, like the level's first checkpoint.
End
The final arena is made out a large set of bone walkways and staircases, mostly submerged in a pool of acid. Moving forward towards the end of the walkway will spawn 8 Filth and 5 Drones. Killing either group will lower the level of the acid down, while killing both lowers the acid enough to make to bottom of the arena accessible. The next wave summoned consists of 4 Strays, 2 Schisms and a Mindflayer. Dealing enough damage to the Mindflayer will also spawn in 3 Soldiers. Once all the enemies are all killed, the elevator will open up and the player can exit the level.
Challenge
The level's challenge requires the player to kill a Mindflayer using acid. This is most readily accomplished by luring the level's first Mindflayer into the flooded Blood Orb tunnel. The orb's overheal can serve as a buffer while the player attempts to outlast the submerged Mindflayer. The Screwdriver may be useful for this challenge, as not only will it provide a constant supply of blood to offset the damage, an embedded Screwdriver drill will also inhibit the Mindflayer's teleportation and prevent it from escaping, though care must be taken to ensure the Mindflayer does not die to the Screwdriver's damage instead of the acid.

Prime Sanctum Entrance
To the far left corner of the Bone Walkways room where the player encounters the first wave of the arena lies a disconnected platform, leading to a room with the third Soul Orb, and a large door. This door is the entrance to P-1: SOUL SURVIVOR, and requires that all the levels in the Prelude and Act I on the currently active save and difficulty to have a P-Rank in order to open.
