First found in 0-2, then introduced in 1-1, Skulls are a reoccurring item in Ultrakill that primarily act as keys to open locked doors. They exist in two colors: blue, and red. Both do not bring any difference as to how they behave except for which corresponding color they can unlock.

Skulls are obtained from Altars of a matching color. When placed down on an empty altar, a skull may activate or open something within the level, generally a door near the player. An exception is made in 7-1, where the player instead finds a red skull resting where the head of a beheaded Mannequin Mannequin would be. If they are to place a blue skull found later in the level on the Mannequin, a wall in the corner of the room will slide open allowing access to a secret corridor that leads to the secret boss of the level.

Red and blue skulls are the only skulls the player will come across throughout the campaign. But a third, purple skull can be seen in 4-S if the player manages to get a third powerup, granting them invincibility and replacing their head with a purple skull.
